  • Identification of the person or organisation (ID number, or business/traffic register number for a company)
  • Vehicle identification (licence number, VIN/chassis, engine number, make and model)
  • Payment reference and amount
  • Banking detail for the refund
  • Signature of the applicant or an authorised proxy or representative
