When you would use the DCT
- A licence disc, registration certificate or notice is lost

- Identification of the person or organisation (ID number, or business/traffic register number for a company)
- The document that was lost and the circumstances
- Vehicle identification (licence number, VIN/chassis, engine number, make and model)
- Signature of the applicant or an authorised proxy or representative
