  • An authority asks for an affidavit before processing

  • Identification of the person or organisation (ID number, or business/traffic register number for a company)
  • Vehicle identification (licence number, VIN/chassis, engine number, make and model)
  • The statement of facts being sworn
  • Commissioner of oaths detail
  • Signature of the applicant or an authorised proxy or representative
